Right-sided congestive heart failure associated with third-degree atrioventricular block and myxomatous valve degeneration in a dog

Abstract
Background:
Myxomatous mitral valve degeneration (MMVD) is the cardiac condition with the highest incidence in small-breed dogs. The main complication of this disease is congestive heart failure (CHF). However, arrhythmias may be observed associated with the progression of this condition. Third-degree atrioventricular block (AV block) is a type of conduction disturbance that can be detected in patients with MMVD. In severe cases of third-degree AV block, animals presenting with an idioventricular heart rate (HR) <100 bpm have a higher risk of developing right-sided CHF.

Case Description:
The present report addresses the case of a male canine, mixed breed, 13 years old, with a history of a systolic murmur at the mitral and tricuspid areas, grade III/VI, and ascites. Diagnosed with four-valve MMVD and third-degree AV block with atrioventricular dissociation.

Conclusion:
The case suggests a possible association between diffuse valvular involvement and the onset of bradyarrhythmias, with progression to right-sided CHF, reinforcing the importance of evaluating the electrical conduction system in patients with multivalvular involvement.
